Medical Xpress reported on July 27, 2026, that a University of Michigan-led Phase III clinical trial found silver diamine fluoride, or SDF, can stop decay in many cavities in young children without drilling, injections or sedation. The study, published in JAMA Pediatrics, tested 38% SDF in 830 children under age 6 recruited through dental offices, pediatric practices, and Head Start and Early Head Start programs in Michigan, New York and Iowa.

According to the report, SDF was applied to affected baby teeth every six months and arrested decay in more than half of treated teeth. The liquid is painted onto a cavity with a small applicator, unlike conventional fillings that require removal of tooth structure. Researchers said the approach could be particularly useful for very young children and for patients who struggle to access or tolerate standard dental care.

Medical Xpress noted that dentists have used SDF in other countries for decades, and it has been used off-label in the United States since 2014 after approval as a medical device for tooth sensitivity. The new trial is intended to provide U.S. safety and efficacy evidence needed for potential Food and Drug Administration approval as a drug to manage dental caries.

The main drawback described is cosmetic: SDF permanently darkens the decayed part of the tooth. Still, researchers involved in the study said broader approval could support wider use, insurance payment and more consistent product quality, potentially reducing pain, infection and surgery for children with untreated tooth decay.
