PBS NewsHour reported July 28 that a new amfAR report describes widespread disruption from cuts affecting USAID and PEPFAR, the U.S. global HIV/AIDS program launched under President George W. Bush in 2003. The segment aired as the International AIDS Conference was taking place in Rio de Janeiro, where the future of HIV funding was a central concern.

According to PBS, amfAR surveyed more than 166 organizations across 46 countries and found that over 1,700 HIV treatment sites had shut down after funding reductions and the broader dismantling of USAID under the Trump administration. PBS said the report also found service interruptions among organizations serving vulnerable groups, including sex workers, men who have sex with men and people who inject drugs, as well as disruptions in programs that help prevent HIV transmission from mothers to children.

William Brangham discussed the findings with Dr. Atul Gawande, a surgeon, writer and former USAID global health assistant administrator under President Biden. Gawande said the figures point to weakening systems for treatment, testing, prevention and outreach to people who may not otherwise seek care. He warned that reduced contact tracing and preventive services could make a resurgence harder to detect, particularly when testing capacity is also diminished.

PBS said the State Department disputed amfAR’s conclusions, arguing that the study’s methodology was flawed and that lifesaving and preventive medicines continue through other partners and arrangements. Gawande responded that newer agreements with governments involve less funding and may not reach marginalized communities as effectively. The issue matters because PEPFAR has been credited with saving more than 26 million lives, and advocates say recent scientific advances could be undermined if delivery systems erode.
