Semafor reports that Sen. John Cornyn, a Texas Republican, is warning he may block two Trump administration ambassadorial nominees unless the administration moves ahead with funding for PEPFAR, the U.S. program created to fight HIV/AIDS abroad, especially in Africa. Cornyn told Semafor he would oppose committee action on Kari Lake’s nomination to serve as ambassador to Jamaica and Doug Mastriano’s nomination for ambassador to Slovakia without assurance that the money will be distributed.

The Senate Foreign Relations Committee is expected to consider Lake’s nomination Wednesday, with Mastriano likely to be included. Semafor reported that the committee has 12 Republicans and 10 Democrats, with Democrats expected to oppose the nominees; Cornyn’s defection could therefore deadlock and sink them in committee if all members are present. Cornyn also said he could vote against the nominees on the Senate floor if the dispute is unresolved.

Cornyn told Semafor he views nomination votes as leverage to press the administration on congressionally appropriated funds. He estimated about $1.3 billion could be released immediately and said he wants clarity on roughly $10 billion appropriated for PEPFAR over the past two years. The administration did not immediately comment, according to the report.

PEPFAR, launched under former President George W. Bush, has long had bipartisan support and is often cited as a major U.S. global health initiative. Semafor noted that the program has faced disruptions during the second Trump administration, including delays and grant terminations despite a waiver for some humanitarian aid. The dispute matters because it links Senate confirmation politics to the future of a high-profile U.S. health and foreign-policy program.
